We have a unique full-time position available on our Home-Based Primary Care Team, where youll make a lasting impact by delivering high-quality primary care directly to patients in their homes.
Since 2013, our specialized team has been serving vulnerable individuals who are unableeither temporarily or long-termto access care in traditional practice settings.

No weekends or holidays
Minimal on-call
Each of the NPs has a caseload of assigned primary care patients within a designated geographic area within our overall service area. Our program serves portions of Albany, Rensselaer, Schenectady and southern Saratoga counties.
The number of patients assigned to each providers caseload is significantly smaller than a typical primary care practice (between 150-185), with approximately five scheduled visits a day, allowing time for travel and longer visits due to the complex needs of our patient population.
NPs start and end their day from home.
Our Home-Based Primary Care Team is supported by:
An experienced collaborating physician
Full-time Practice RN/Care Manager
Full-time Office Manager
Access to support from pharmacy, social work, care management, and more!
Our team is co-led by our health systems Continuing Care Executive VP and the Executive Director of our IHANY Accountable Care Organization, providing access to resources and assistance as needed

REQUIREMENTS:
Applicants should have the following:
Board Certified- Adult Gerontology or Family Nurse Practitioner Certification / Internal Medicine
NYS License and DEA certification
At least 2-3 years of Nurse Practitioner experience working with an elderly population is required.
Experience working as a Nurse Practitioner in a Primary Care setting is preferred.

RECRUITMENT PACKAGE
St. Peters Health Partners offers a comprehensive salary and compensation package that includes:
Compensation range: $124,800- $139,360 plus on-call pay and mileage reimbursement
The posted salary reflects the starting range of total compensation and does not include productivity bonuses, incentives related to quality and performance, extra shift incentives, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer. Please note that this salary range is provided in accordance with State law and is subject to variation due to the factors noted above.
